ManyChat DM automation for a medical practice does one job: it makes sure every person who asks a question on Instagram gets an answer in seconds, and a booking link while they still care. That is where a lot of the money from social media is won or lost. I run RobbP Media, a social media and paid ads agency in Los Angeles, and on Dr. Andrew Brunswick's account the ManyChat setup built a list of more than 54,750 contacts while his Instagram grew from 9,590 to 233,000 followers on organic content alone. This is how I set it up for practices, and what I would not automate.
Why DMs are where practices lose patients
A person who comments "how much?" under a Reel, or sends a DM at 10pm asking whether you treat their problem, has raised a hand. And in most practices that message sits in an inbox nobody owns until someone at the front desk gets to it the next afternoon. By then they have moved on.
A surgeon cannot answer DMs between patients, and a busy front desk cannot either. DM automation closes that gap. It does not replace your team; it answers the easy part instantly so your team spends time on the people ready to talk.
What ManyChat actually does for a practice
ManyChat connects to your Instagram and Facebook inbox and replies based on rules you set. The three pieces I use on almost every practice account:
Comment to DM. A Reel ends with "comment BOTOX and I will send you our pricing and the booking link." Everyone who comments that word gets a private message within seconds. It turns a public view into a private conversation.
Keyword and FAQ replies. The ten questions your front desk answers every day, such as cost, consult fee, location, parking, recovery time and financing, get short, accurate answers written in the practice's voice and approved by the doctor.
Qualify and hand off. A couple of quick buttons ask which treatment the person is interested in and how soon they are thinking about it. Then they get the booking link, or the conversation is tagged and passed to a real person. The consult request arrives already labeled by procedure, which is the part front desks love most.
How I build the flows
I start with the questions, not the software: we list what people really ask the front desk, in their words. Then I write one flow per high value treatment, because a rhinoplasty enquiry and a facial enquiry need different next steps.
Each flow follows the same shape. Answer the question they asked, briefly. Offer one clear next step, usually the booking link. Give them a way to reach a human with a single tap. Three messages, not twelve. Long automated sequences feel like a phone tree, and people leave phone trees.
Then we connect the back end. Leads from ManyChat go into GoHighLevel, so the team sees every conversation in one place and anyone who has not booked gets a follow up by text or email. This is the same system behind our booked consults program, where the ads, the landing page and the follow up are built as one piece.
What I would never automate
Medical advice. A bot should never tell someone whether a treatment is right for them or what result to expect. That comes from a clinician, in a consultation.
Sensitive details. Instagram DMs are not the place to collect health history, so the flows never ask for it. If someone starts sharing personal medical information, the reply moves them to a phone call or your own intake form. Check your setup with whoever handles compliance for the practice; I am not a lawyer, and the rules differ by specialty and state.
Anything emotional. Complaints, unhappy patients and people who are clearly anxious go straight to a person. The fastest way to damage a practice's reputation is a cheerful bot answering a frustrated message.
The content that feeds the flows
Automation only works if people are messaging you, and people message accounts that answer real questions on camera. On Dr. Brunswick's account the videos were the surgeon explaining what patients ask, in short Reels that fit around his schedule, and ManyChat turned the comments into conversations. You can read the full story in the Dr. Andrew Brunswick case study. The same pattern works across specialties, and I wrote up how it applies in Instagram for dentists and in how a medical practice grows on Instagram without buying followers.
Paid traffic benefits too. When we run Meta lead campaigns for a practice, the people who click into a DM instead of a form get the same instant reply. For Dr. Zoumalan, following every lead through to a booked consult was a big part of how $1,837 of Meta spend turned into $24,500 in revenue from 43 qualified leads.
How to tell if it is working
Count three things each month: conversations started, booking links clicked, and consults booked that came from a DM. The ManyChat dashboard gives you the first two. The third needs a tracked booking link and a "how did you hear about us" question on your intake form. If conversations are high and bookings are low, the flow is answering questions but not giving a clear next step. If conversations are low, the content is not asking people to message. For the wider list, see the social media metrics that matter.
Doing it yourself or hiring help
A practice can build a simple comment to DM flow in an afternoon. Where it stalls is the writing, the calendar and CRM connection, and the monthly tuning against real conversations. That is the work we take on in our Instagram growth agency service, and our medical aesthetics marketing work covers the full system for plastic surgeons, med spas and dermatology practices. Management starts from $1,500 a month.
Questions people ask
Is ManyChat worth it for a medical practice?
Yes, if people already comment on or message your account. ManyChat answers them in seconds, gives them a booking link and passes serious enquiries to your team. If nobody is messaging yet, start with the content, because automation has nothing to answer.
Is ManyChat HIPAA compliant?
Do not treat Instagram DMs as a place to collect protected health information. We build flows that answer general questions and send people to a phone call or your own intake form for anything personal. Ask your compliance adviser to review the setup before it goes live.
Will patients know they are talking to a bot?
They should. We write replies in the practice's voice but make it clear when a message is automatic, and every flow has a one tap way to reach a real person. People mind slow answers far more than they mind a quick automatic one.
How long does ManyChat take to set up?
A first comment to DM flow can be live in a day. A full setup with flows for each main treatment, qualifying questions and a connection to your calendar and CRM usually takes a couple of weeks, then gets tuned every month against real conversations.
Can ManyChat book appointments directly?
It can send a booking link and pass details to your scheduling tool or CRM, so the person picks a time without waiting for a call back. We usually connect it to GoHighLevel so anyone who does not book gets a follow up.
Let's talk
If your practice gets DMs that sit unanswered, or you want comments on your Reels to turn into consults, email me at info@robbp.com. Tell me which treatments you want more of and how your front desk handles messages today, and I will tell you honestly what I would build first.
